Requesting Prescriptions

We are making ordering repeat prescriptions easier so please complete the form below to email your request to the surgery.

We require at least 2 working days notice to process a repeat prescription request. For your convenience, and due to current COVID restrictions, we still have a secure courier service delivering prescriptions to all the Island's pharmacies, so please be sure to include where you wish your prescription to be sent to.

When COVID-19 restrictions ease we will be reverting to our previous online service, so why not sign up now for when this service is back up and running. This is a great way to request your repeat prescriptions, but also allows you to book, change and cancel appointments online too. For more information, please review our 'Online Services - Practice Policy'. To register for this service, download and complete the registration form found below, and return to the surgery.

Please note: in order for us to provide you with your online login details, we will require to see proof of photo ID and address. This is to ensure we are registering and linking the correct person to use this service. Thank you for your cooperation in this matter.
