Keeping your personal information up to date ensures that we can provide you with accurate service should we need to make contact with yourself. If any of your personal details have changed - such as your name, address, contact number, email address or next of kin, we kindly ask that you notify us as soon as possible.

Please complete the form and return this into our surgery reception along with Proof of ID for our staff to amend. Alternatively, you are able to update your details through the one.gov.je website please make sure you select Cleveland Clinic as your GP so we get notified.
