How to make an appointment

-Please complete a form found below or collect from our surgery

- Drop the form into the reception or send this back into us.

-Our travel team and travel GP will have a look into your destination and which injections you may need depending on your immunisation history. We ensure you are up to date with any vaccinations before your travel.

-We shall be in contact with you to arrange an appointment for any vaccinations that you are due, if any. You will then be booked in to a set clinic to see our travel GP.

-We recommend you complete this form/ have an appointment book at least 6-8 weeks before your departure.
