Make sure your child is registered with the same clinic as you!

We welcome the opportunity to care for your child's health, offering a caring, family-focused environment where your child's health and well-being are our top priority. Children under 16 years must be registered by a parents or legal guardian and can only be registered with one surgery on the Island.

Please complete the below form and return this to our reception team in person. You will be asked to provide proof of ID and address for you child or new born.

Once registration is complete, we may invite your child for a new patient health check with one of our clinicians. This helps us get to know your child's health and ensures continuity of care. You're new born shall then be invited to our immunisation clinics in due course.
