    Sharing Your Health Information

    Protecting your sensitive medical details.

    At Cleveland Clinic, we take your privacy and confidentiality very seriously. By Law, we cannot and will not share any personal information or details about your medical care with anyone, this includes close family members and friends, unless we have your explicit consent to do so complying with the Data Protection Act.

    If you would like your medical records and sensitive data to be discussed with a family member, friend or carer, we would require you to give your consent in writing. This applies to 16+ years and over.

    Sharing specific information

    What information can be shared?

    With your permission, we can share information such as:

    - Appointment details and cancellations

    - Test results

    - Medications and treatment plans

    - Updates on your health and/or ongoing care

    - Speaking with your GP on your behalf

    Please be advised should you wish for your information to not be discussed with other medical professions like the Hospital or Family Nursing, please do let us know as we shall need you to complete an OPT - OUT Form.

    You can update the person of contact or withdraw your consent at any time. Just let us know, and we shall make the necessary changed to your records.
