Requesting your records?

At Cleveland Clinic, we are committed to transparency and respecting your rights to access your personal information. Under data protection laws, including the General Data Protection Regulation (GDPR), you are entitled to request your medical information that we hold about you.
Access to your data is free of charge. However, we may charge a fee if your request further copies of the same information.

How long will access to my records take?

We legally have one month to respond to your request from the date of receiving it. In some cases, if the request is complex or involved a large volume of information, we may have to extend this period but we shall let you know if this is the case.

How to access my records?

-Please complete the below Subject Access form with as much detail as possible, and return this into the reception team with proof of ID.

- Our team shall look into your request and prepare your medical records

- Processing of signing off and finalising

- Our team shall be in contact with you to arrange collection. Please note we shall try our best to meet the needs of collection or email however it depends on the size of your file. For secure reasons it is recommended this is collected by yourself with proof of ID to ensure confidentiality is maintained.
